DOP75 Video capsule endoscopy may detect active small bowel Crohn’s disease requiring change of treatment despite unremarkable ileo-colonic anastomosis

Abstract Background Previous studies have shown benefit in performing video capsule endoscopy (VCE) Crohn’s disease (CD) diagnosis, especially cases of an unremarkable ileo-colonoscopy, however data on the role VCE postsurgical follow-up CD patients is lacking. We aimed to evaluate endoscopic activity small bowel (SB) presenting with remission at ileo-colonic anastomosis. Methods retrospectively obtained older than 18 years age, who underwent ileo-colonoscopy between dates 1/10/2012 through 31/09/2022. Patients whom was performed within 8 months were eligible for analysis. Lewis and Rutgeerts scores calculated based images reports, respectively. Disease deemed as score i0-i1 anastomosis, (LS) of<135 VCE. Results A total 1,334 ileo-colonoscopies anastomoses during study period, 31 (male gender 58.06%; mean age 41.77±14.19 years) referred (median 124.81 days). Twenty exhibited 3 them failed passage a patency 17 successfully completed Only 5 (29.4%) had normal LS, while 12 (70.6%) active SB (LS≥135, 791.3±1254.9). discrepancy two scores, characterized by shorter duration (P=0.011) showed trend towards time since surgery (P=0.06). No correlation found concurrent medical therapy (P=0.898). Treatment modification applied 7 (41.2%) associated higher LS (biserial coefficient 0.51, P=0.037). Conclusion Endoscopic anastomosis not accurate indicator SB, new-onset soon after surgery. has added value treatment adjustment CD.
